Entries must be sent to BAM at email jane@bam.org.my or fax at ++603 2094 5855 with the attached ENTRY FORM (please indicate the player’s BWF no.).

Following entries are NOT acceptable:

1. Received after the closing date;

2. Without authorised signature of the national association;

3. Made on an individual basis.

- Seedings and Draws will be conducted by BWF.
- Withdrawal
        - Withdrawals by players will be handled under the provisions for the World G.P. Regulations – PENALTY FOR WITHDRAWAL.
   - Doping Testing will be conducted at random throughout the tournament.

The deputy referee of the Championship is Mr. Chua soo hock(MAS).

All players and officials, who require visa to enter Malaysia should submit full passport details (Full name, Passport No., and Date of Issue & Date of Expiry, contact no.) to email jane@bam.org.my or fax ++603 2094 5855 by 15 December 2007.

Room Rates:Single/Double Room – RM 270.00 (approximately US$78.00) per room/night (inc. Breakfast).

Reservations:Association are requested to email the hotel reservation form complete with rooming list, full name, passport number and date of birth not later than 15 December 2007 to email jane@bam.org.my and cc to yannitan@pbs.mines.com.my, or fax at ++603 2094 5855 and ++603 8943 5555.

Payment:No guarantee can be given with respect to late booking.  50% non-refundable deposit of the total amount should be paid by the 20 December 2007.  You may modify the number of rooms, number of night of stay and participant’s name without penalty before the 28 December 2007.

Final 50% payment must be made before 4 January 2008.Payment must be made by bank transfer to the following bank details:

Any person who would check-out from the hotel before the date initially planned, 1 additional night will be charged.
